I did not receive the instructions, and apparently those who forwarded other
questions didn't get it either. 

I think the appropriate response would have been to resend those
instructions or confirm that we had them.

I received a hotel confirmation on ICANN letterhead on Jan 17th, along with
a very detailed description of how to complete the visa application (copy
attached here). I think that this is the guide referred to in red on your
wiki page.

The only thing that was not crystal clear to me was what to fill in
3.6 on Major Family Members. I thought that it was only for family
travelling with you, but they want just what they are asking for. 
Specifically, if earlier say you are married, you should include your spouse
here.
