Dear Summit WG members:

As requested, here is a draft survey which would be sent to those respondent
ALSes who are eligible for travel support to attend the Summit, to
establish:

* Essential information for booking travel support
* Choosing in order of preference the subjects to be tackled by the five
policy working groups
* Providing updated information about each ALS, its activities, and
membership
* Determining if they are willing to provide printed or audiovisual material
for the poster / materials exhibition at the Summit

I note that the popup text for each of the subjects in the list of topics
for the working groups is not included. The idea is that these would provide
short easy to understand explanations of what the subjects mean - I would
propose to create these once the Summit working group has decided upon the
final list of topics in the survey.

If it is at all possible, we really need the working group to finalise the
text of the survey at the meeting this Friday in order to get translations
done, and the survey setup and tested quickly.
